What is Prednisolone?

Prednisolone is a synthetic glucocorticoid, remedy a derivative of cortisol, which is used to treat a variety of inflammatory and auto-immune conditions. It is the active metabolite of the drug prednisone[1] and is used especially in patients with hepatic failure, as these individuals are unable to metabolise prednisone into prednisolone.

What is the mode of action?

Prednisolone irreversibly binds with glucocorticoid receptors (GR) alpha and beta for which they have a high affinity. AlphaGR and BetaGR are found in virtually all tissues with variable numbers between 3000 and 10000 per cell, depending on the tissue involved. Prednisolone can activate and influence biochemical behaviour of most cells. The steroid/receptor complexes dimerise and interact with cellular DNA in the nucleus, binding to steroid-response elements and modifying gene transcription. They induce synthesis of some proteins, and inhibit synthesis of others.

Not all metabolic actions on genes are known. Most mediator proteins are enzymes, e.g., cAMP-dependent kinase

Anti-inflammatory and immunosuppressive actions:

Inhibition of gene transcription for COX-2, cytokines, cell adhesion molecules, and inducible NO synthase

Blockage of Vit D3-mediated induction of osteocalcin gene in osteoblasts

Modification of collegenase gene transcription

Increase synthesis annexin-1, important in negative feedback on hypothalamus and anterior pituitary gland

Regulation of gene suppression leads to systemic suppression of inflammation and immune response. This is of clinical usefulness but ultimately leads to gluconeogenesis, proteolysis and lipolysis. Gene transcription returns to normal after cessation, but sudden stoppage can cause Addison’s disease. Osteoporosis is permanent.

What are the side effects?

Short Term Side Effects of Prednisone:

Some of the short term prednisone side effects include insomnia and mania may also manifest itself in patients that have been diagnosed with Bipolar Disorders. Other short term side effects of prednisone include high blood glucose levels, fluid retention.

Long Term Side Effects of Prednisone:

Some of the long term prednisone side effects are truncal weight gain, osteoporosis, glaucoma and cataracts. When dose reduction takes place other side effects include Type II Diabetes and depression.

Major Prednisone Side Effects:

There are a few major side effects as a result of taking prednisone. The side effects include, but is no means limited to, difficulty in controlling emotion and train of thought, weight gain and facial swelling, unusual weakness or fatigue, indecisiveness and blurred vision, abdominal pain and peptic ulcer. Long term migraines, joint pain and anxiety are a few other long term side effects of prednisone.

Usage and Administration

Your dose of prednisolone may consist of several tablets. This will be written clearly on the label or the instructions you have been given. Unless you have been told otherwise, take this dose all together with your breakfast.

Take prednisolone tablets with food unless you have been prescribed the enteric-coated (red or brown) tablets. The enteric-coated tablets may be taken before or after food.

If you have been given enteric-coated prednisolone, swallow these tablets whole. Do not chew or crush them. You should avoid taking indigestion remedies at the same time as enteric-coated prednisolone as these can interfere with the special coating on your tablets.

If you have been given soluble prednisolone tablets, you must dissolve or mix these in water before swallowing them.

If you have been taking prednisolone for more than 3 weeks, do not stop your treatment suddenly. If you need to stop taking prednisolone, speak with your doctor who will advise you on how to reduce your dose gradually.

You need to take prednisolone regularly to get the maximum benefit, so try take your doses at the same time each day to avoid missing any doses.

If you do forget to take a dose, take it as soon as you remember unless it is nearly time for your next dose, in which case leave out the missed dose. Do not take two doses together to make up for a forgotten dose.

Storage

Keep all medicines out of the reach and sight of children. Store in a cool, dry place, away from direct heat and light.
