What is Dexamethasone?

Dexamethasone is a potent synthetic member of the glucocorticoid class of steroid drugs. It acts as an anti-inflammatory and immunosuppressant. When taken orally, it is 26.6 times more potent than the naturally occurring hormone cortisol and 6.6 times more potent than prednisone.

What is the mechanism of action?

Dexamethasone is a glucocorticoid agonist. Unbound dexamethasone crosses cell membranes and binds with high affinity to specific cytoplasmic glucocorticoid receptors. This complex binds to DNA elements (glucocorticoid response elements) which results in a modification of transcription and, hence, protein synthesis in order to achieve inhibition of leukocyte infiltration at the site of inflammation, interference in the function of mediators of inflammatory response, suppression of humoral immune responses, and reduction in edema or scar tissue. The antiinflammatory actions of dexamethasone are thought to involve phospholipase A2 inhibitory proteins, lipocortins, which control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes.

How to take Dexamethasone?

• Before beginning treatment, read the manufacturer’s printed information leaflet and be careful to follow the printed instructions you have been given with these tablets.
• Take dexamethasone exactly as directed by your doctor.
• Take your dose of dexamethasone with or immediately after a meal. It is best taken in the morning with your breakfast unless you have been told otherwise by your doctor.
• Take dexamethasone at the same time each day to avoid missing any doses.
• If you do forget to take a dose, take one as soon as you remember unless it is nearly time for your next dose in which case skip the missed dose. Do not take two doses at the same time to make up.
• If you have been taking dexamethasone for more than 3 weeks it is important that you do not stop taking it suddenly without speaking to your doctor first. Your dose will need to be reduced gradually, as stopping suddenly may cause problems.

• vision problems
• cold or infection that lasts a long time
• muscle weakness
• black or tarry stool

Storage
