What is Darunavir?

Darunavir is a drug used to treat HIV infection. It is in the protease inhibitor class. Darunavir is an OARAC recommended treatment option for treatment-naïve and treatment-experienced adults and adolescents. Developed by pharmaceutical company Tibotec, diagnosis darunavir is named after Arun K. Ghosh, the chemist who discovered the molecule at the University of Illinois at Chicago. It was approved by the Food and Drug Administration (FDA) on June 23, 2006.

What is the mechanism of action?

Darunavir is a HIV protease inhibitor which prevents HIV replication by binding to the enzyme’s active site, thereby preventing the dimerization and the catalytic activity of the HIV-1 protease. Darunavir selectively inhibits the cleavage of HIV encoded Gag-Pol polyproteins in virus-infected cells, which prevents the formation of mature infectious virus particles. Structual analyses suggests that the close contact that darunavir has with the main chains of the protease active site amino acids (Asp-29 and Asp-30) is an important contributing factor to its potency and wide spectrum of activity against multi-protease inhibitor resistant HIV-1 variants. Darunavir can also adapt to the changing shape of a protease enzyme because of its molecular flexibility. Darunavir is known to bind to two distinct sites on the enzyme: the active site cavity and the surface of one of the flexible flaps in the protease dimer.

How to dose Darunavir?

The recommended oral dose of darunavir tablets is 600 mg (two 300 mg tablets) twice daily (BID) taken with ritonavir 100 mg BID and with food. The drug can be taken with any type of food.

What is the side effects?

Medicines and their possible side effects can affect individual people in different ways. The following are some of the side effects that are known to be associated with this medicine. Just because a side effect is stated here does not mean that all people using this medicine will experience that or any side effect.

  • Disturbances of the gut, such as diarrhoea, indigestion, nausea and vomiting, abdominal pain, flatulence.
  • Headache.
  • Increase in the level of cholesterol and triglycerides in the blood.
  • Dizziness.
  • Fatigue, weakness or loss of strength (asthenia).
  • Difficulty sleeping (insomnia).
  • Skin reactions such as rash, itching, hair loss, acne, dry skin, sweating.
  • Pins and needles or numb sensations.
  • Balance problems involving the inner ear (vertigo).
  • Increased blood pressure.
  • Fever.
  • Shortness of breath (dyspnoea).
  • Dry mouth.
  • Anxiety, irritability, confusion and memory problems.
  • Swelling of the legs and ankles due to excess fluid retention (peripheral oedema).
  • Increased blood glucose level (hyperglycaemia).
  • Pain in the muscles or joints.
  • Erectile dysfunction (impotence).
  • Redistribution of body fat, eg decreased fat under the skin, increased fat on the abdomen, ‘buffalo humps’ and breast enlargement.
  • Alteration in results of liver function tests.
Storage

Store darunavir at 77 degrees F (25 degrees C). Brief storage at temperatures between 59 and 86 degrees F (15 and 30 degrees C) is permitted.
